Membership & Bylaws

 

To become a member of the Quinlan Band Boosters Association, you must complete the following documents:

 

 

These completed forms, along with a copy of the applicant's valid driver's license, need to be returned to a Booster officer or Band director.

 

The QISD forms and the driver's license copy will be turned into QISD Human Resources. The Booster Application will be kept by the Booster Vice President. The information you provide will only be used by the Boosters and Band directors and will not be shared. By filling out these forms, you are giving permission for the Boosters to contact you regarding Booster activities, including meetings and volunteer opportunities.
